General Prescription Drug Enrollment Period for people who are new to Medicare
People who are new to Medicare have a 7-month window to enroll in a Prescription Drug Plan. Your effective date for Medicare is the first day of the month of your birthday. You can enroll in a plan 3 months before your birthday month, the month of your birthday, and up to 3 months after your birthday month.
After your receive your Medicare card, you can come to the senior center to get a form we need completed to do a comparison of plans. The form will have a space for your personal information: Name, Address, Phone, Birthdate, Medicare claim number, Medicare effective date. It has a space with qualifications for Extra Help paying for your prescriptions. If you think you qualify by Household income and assets, please give us those amounts. Fill in your preferred pharmacy name.
You will need to list the Prescription Medicines you take on the back side of the form. List the name of the medicine you buy, generic or brand name. If you must take the brand name, indicate that in the Brand Name Only column. List the dosage and the number you buy each month, whether it be pills, capsules, tubes, vials, boxes, etc.
Leave the form with us, so a counselor can do a comparison. We will call you to set an appointment when the comparison is complete.
